Forklift driver's license (forklift license) is the People's Republic of China (PRC) special operation certificate issued by state administration of work safety. Forklift is only used in the workplace of this unit.

Before taking the forklift license, you must first learn to drive a forklift, and you must be trained through a formal training institution. Then the materials submitted by the training institutions will be assessed by the personnel of the Municipal Safety Supervision Bureau, and the forklift license can only be obtained after passing the theoretical and practical operation examinations.

(2) Application conditions and materials:

2. At least 18 years old and in good health;

3. Abide by discipline, love your job and have a strong sense of responsibility.

Above must bring my ID card, academic certificate (one for each), four recent bareheaded two-inch color photos, and a certificate of safety education, training and internship from the Quality Supervision Bureau.

(3) Examination fee:

1. If you need pre-test training, please ask the relevant training units for training fees and training materials.

2. The registration unit is: the special equipment safety supervision section of the local quality and technical supervision bureaus will register first, and the unified examination will be conducted after reaching a certain number.

3. The examination is organized by the examination center for special equipment operators.

4. The examination fee shall be charged according to the local "Charging Standard for Safety Skills Examination for Operators of Special Equipment", and the specific fee shall be subject to consulting relevant training institutions.

At present, China has opened an online inquiry system for special work permits, that is, comrades who need to inquire about information can find the license information by entering their names, ID numbers and ID numbers. The certificate is reviewed every three years and renewed every six years. It can be reviewed and renewed all over the country, which is universal throughout the country.

(5) Review conditions:

One of the following circumstances is unqualified, and it is unqualified for review:

1, in good health, without diseases and physical defects that hinder the operation of the corresponding type of work;

2. No production safety accidents occurred during the review period;

3. Apply for review within the review time specified in the certificate;

4, review must participate in the review knowledge update training, and pass the exam;

5, the age is not more than 60 years old.

(7) The time required for the review shall be completed within 2 working days after receiving the review materials submitted by the training institution and audited by the provincial safety supervision bureau, and the longest time shall not exceed 35 working days.

(eight) the validity period and review period of the certificate.

1, the certificate is valid for 6 years;

2, special personnel operation certificate review once every three years;

3, special operations personnel review must participate in knowledge update training, after passing the examination, the replacement (replacement) procedures;

4. Special operations personnel shall apply for reexamination to the reexamination institution 3 months before the expiration of the reexamination period indicated in the certificate, and the overdue certificate will automatically become invalid.
