Transcription of medical records should reflect the following parts:

According to teacher Wang Jian's request, all the reports should be written within 72 hours. If any of them are not completed, they must go back and do it again. The patient can't be diagnosed.

This part of medical record transcription needs introduction, which only accounts for a small part of the whole practice teaching content and takes less than 30 minutes, but I feel it is particularly important for our doctors. It is the main idea of doctors' diagnosis, in other words, it is a question of how far your doctor's idea is put into practice before you can make a qualitative diagnosis of the immediate case. Whether the medical records are rigorous or not can reflect the level of a hospital and the medical skills of doctors. It is also directly related to the accuracy of diagnosis.

In the previous textbooks, it was simply talked about understanding the general situation, physical symptoms, psychological symptoms and family support system structure of the help seekers to assist the scale evaluation. However, the introduction in the book is not as comprehensive as that of Professor Wang. Professor Wang clearly emphasized the following points in class.

In family history, parents should be asked about the marriage of close relatives of two families and three generations.

In the part of mental examination, we should pay full attention to checking the patient's self-control.

The above points are not included in the textbook, nor are they emphasized. Those parts are very important in clinic. According to the idea of copying medical records introduced by the teacher, it gives people a serious, earnest, scientific and rigorous feeling, embodies the scientific spirit of people and deserves a responsible attitude towards patients.

When Professor Wang invited patients for clinical teaching in combination with medical records, the points described in the medical records basically covered the patients' situation in an all-round way, even more than the patients knew (because the patients' parents added them in the medical records), which obviously surprised the patients.
