Forklift driver's license (forklift license) is the People's Republic of China (PRC) special operation certificate issued by state administration of work safety. Forklift is only used in the workplace of this unit.

Before taking the forklift license, you must first learn to drive a forklift, and you must be trained through a formal training institution. Then the materials submitted by the training institutions will be assessed by the personnel of the Municipal Safety Supervision Bureau, and the forklift license can only be obtained after passing the theoretical and practical operation examinations.

Application conditions and information:

1, junior high school education or above;

2. At least 18 years old and in good health;

3. Abide by discipline, love your job and have a strong sense of responsibility.

Above must bring my ID card, academic certificate (one for each), four recent bareheaded two-inch color photos, and a certificate of safety education, training and internship from the Quality Supervision Bureau.

According to Article 13 of the Management Regulations on Safety Technology Training and Assessment for Special Operations Personnel, the personnel who take the qualification examination for special operations shall fill in the examination application form, and the applicant or the unit where the applicant belongs shall apply to the examination issuing authority or its entrusted unit where the applicant's domicile or employment is located with the academic certificate or the training certificate issued by the training institution.

After receiving the application, the examining and issuing authority or its entrusted unit shall organize the examination within 60 days.

The special operation qualification examination includes two parts: the safety technology theory examination and the practical operation examination. Those who fail the exam are allowed to make up the exam 1 time. If the make-up exam is still unqualified, re-participate in the corresponding safety technical training.
