1. The medical examiner should fill in the Health Checklist for Staff in Nursery Institutions with the ID number above, paste the recent electronic photos on the form, print the medical check form and confirm the signature; During the physical examination, you should carry the printed physical examination form, ID card and health certificate with you. The staff who have applied for a new card or replacement card must submit a recent one-inch bareheaded formal photo to the information desk of the physical examination center.
