Focusing on the three major objectives of "changing ideas, forming habits and improving abilities" and the three major tasks of "HSE system standards, training and performance assessment", with the "three basics" work as the core and the goal of continuously improving the long-term mechanism of HSE management, the safety management in the operation area has achieved remarkable results and the HSE management level has steadily improved. The safety activities in the past year are summarized as follows:

First, strictly implement the "seven plans" of oil production plants and strengthen basic management.

(1) Earnestly implement the HSE work promotion plan of oil production plant 20 12, promote system construction, and continuously improve the operation level of HSE system.

1. Revise system documents and articles of association. According to the arrangement and requirements of the Implementation Plan for HSE Work in Oil Production Plant 20 12, the HSE management manual in the operation area was revised in August 20 12. In order to verify the suitability and effectiveness of the HSE system in the operation area and check the compliance and operability of the system documents, internal audit was carried out through discussion, receiving information, on-site inspection and summary, and suggestions were put forward for the problems found in the system operation. According to the geographical span of the work area and the complicated domestic traffic situation, the special identification and evaluation of road traffic risk in the work area are formulated, and the road conditions of different sections are identified, and the preventive measures for risk reduction are formulated, which further improves the identification of risk factors in the work area.

2. Revise the management system and operating procedures. The operation area is in the rising period of development, and the personnel organization and operation scope are constantly changing and expanding. At the beginning of 20 12, the management organization of the operation area was greatly adjusted, three oil production teams were established, and safety offices and offices (party and government) were added, and the functions of each office changed accordingly; A series of changes require all management systems in the operation area, and the responsibilities and post responsibilities of all departments should also keep pace with the times and keep up with the development pace of the operation area. The safety office in the operation area shall cooperate with relevant departments to revise and improve various management systems, and modify the responsibilities of various departments and posts from reality. According to the requirements of the document, combined with the safety and environmental protection situation and work priorities in the operation area, the work report of the 20 12 HSE meeting in the operation area, the promotion plan of the 20 12 HSE management in the operation area, and the annual assessment rules of the HSE management in the operation area are formulated, so that all safety and environmental protection work can be carried out in an orderly manner.

4. Implement personal safety action plan. Managers and team leaders at all levels in the operation area have worked out 49 personal safety action plans according to the principle of combining territorial management and post linear responsibility with work practice, and published 32 personal safety action plans on the Internet every quarter according to the requirements of the Notice on Publicity of Personal Safety Action Plans, which has played a positive role in vigorously promoting the concept and practice of "induced leadership" and "linear responsibility" and realizing the transformation of "three views"

(2) Implement the work plan for the investigation and management of safety and environmental hazards in 20 12 oil production plant, and strengthen the source management and dynamic process supervision.

The operation area has formulated the 20 12 annual safety and environmental protection hidden danger investigation and management plan, and cooperated with a series of safety hidden danger management activities such as "cracking down on illegal activities", "special equipment hidden danger investigation" and "safety hidden danger self-inspection" in oilfield companies and oil production plants this year, and carried out inspection activities of different levels and categories such as operation area inspection, grass-roots team self-inspection, monthly inspection, weekly inspection and daily inspection, and passed the inspection.

Safety work focuses on the grass-roots level. At the scene, routine patrol inspection is the main means to find problems. Only by facing the scene, paying close attention to implementation, strengthening on-site safety supervision and inspection, urging and helping to solve on-site safety problems can we fundamentally promote basic safety. In the form of holiday safety inspection, special safety inspection, daily safety inspection and fixed-point inspection in the contracted responsibility area, the basic safety inspection was carried out in the operation area, which effectively promoted the continuous stability of basic safety.

1, carry out daily safety inspection continuously and carry out various special safety inspections. According to the requirements of the superior, combined with the actual production and climate characteristics, we carried out weekly and monthly daily safety inspections, as well as pre-holiday safety inspection, fire inspection, oil loading and unloading platform inspection, gas unit inspection, food hygiene and other special inspections for 8 times, and found 88 hidden dangers every year, followed up and rectified 88. At present, the output rate of rectification has reached 100%. On the basis of investigation and evaluation of hidden dangers, supervise and urge the rectification and review of hidden dangers and problems found, and eliminate hidden dangers in production in time.

2. Intensify the investigation of the three violations and analyze the reasons for the three violations. This year, the higher authorities investigated and dealt with five violations in the operation area, including four in the oilfield company and one in the oil production plant 1. The five violations investigated and dealt with by superiors all occurred in 1 quarter, and were investigated and dealt with by superiors at the beginning of the year, which sounded the alarm for the safety work in the operation area. We attach great importance to this work area, carefully analyze the problems, re-equip full-time safety management personnel, strictly urge personnel at all levels to carry out self-inspection, intensify the inspection of three violations, and severely investigate and deal with three violations. Through a series of effective measures, the safety of the work area has been greatly improved. In the second and third quarters, the work area was not investigated by the superior unit because of a case of three violations.

This year, 40 cases of "three violations" of oilfield companies, oil production plants and operation areas were investigated and dealt with, of which 4 cases were investigated and dealt with by the Safety and Environmental Protection Supervision Center of oilfield companies, accounting for10%; Investigation by safety supervision station of oil production plant 1, accounting for 2.5%; 35 cases (87.5%) were investigated by self-examination in the operating area.

2), the causes of "three violations":

(1), the source control is not strict, the design, construction and acceptance are not strict, and the necessary safety facilities are not equipped according to the relevant safety specifications, resulting in the phenomenon of "three violations" after production;

(2) The safety supervision and inspection personnel do not pay enough attention to the on-site safety work, and can't carry out supervision and inspection in combination with the actual production and the anti-"three violations" work of oilfield companies and oil production plants;

(3) Grass-roots teams did not seriously study the "three violations" reported by oilfield companies and oil production plants, and did not conduct mutual inspection of "three violations";

(4) The management and supervision personnel of the contractor are not familiar with the relevant safety standards and inspection specifications, and cannot find and eliminate violations and hidden dangers.

(3) Implement the 20 12 environmental protection work plan of the oil production plant, and strengthen environmental protection and governance.

The operation area has formulated assessment and control measures for environmental risk factors, strengthened environmental management, and continuously carried out green oil and gas field construction and cleaner production activities.

Improve the environmental protection document system and organization. The laws and regulations applicable to the creation of green oil and gas fields and environmental protection, as well as the relevant system documents of group companies, oilfield companies and oil production plants were screened out, and two activity plans were formulated according to the needs of the operation area, so that the activities could be carried out with goals and the inspection and acceptance could be based. A leading group for creating activities was set up, with the manager of the operation area as the first person responsible for environmental protection, coordinated by the production operation office, in charge of safety, and the participation of various departments and grass-roots teams, forming a good atmosphere of top-down * * * management and joint management.

Strengthen the treatment of three wastes. Do a good job in the supervision and management of workover site, and do a good job in blowout prevention, leakage prevention and seepage prevention. Throughout the year, 87 wells were operated 187, and impervious cloth was laid 13 1584 square meters. (4) Implement the traffic safety work plan of oil production plant 20 12, and strengthen traffic safety management.

Traffic safety has always been the top priority of work area safety work. The geographical span of the work area is large, the oil areas are scattered, the national highways and provincial highways are intertwined, and the road traffic situation is complicated. The leaders in the work area pay special attention to traffic safety. The traffic safety work in the operation area is comprehensively managed from several aspects, such as strengthening personnel training and education, strictly checking before leaving the vehicle, strictly observing the traffic three-inspection system, and strictly checking the unsafe driving behavior of employees by road inspection.

Organize regular training. According to the 20 12 HSE training plan in the operation area, vehicle drivers are regularly organized to learn about traffic season change and recent typical traffic accident cases, and traffic safety special study and seasonal traffic training are organized 13 times a year, with a total of 2 17 participants.

Strictly abide by the three-way traffic system. In the whole year, 9 cases of grass-roots departments failing to implement the three communications according to the regulations were investigated and dealt with, mainly in the first half of the year.

Strict inspection before leaving the car. The safety office and operation office in the operation area regularly go to the parking lot to give safety lectures on vehicles before they leave the car, conduct spot checks on vehicle condition inspection, and supervise grass-roots teams to implement the daily inspection system.

Severely investigate and deal with road violations. Focus on rectifying employees' unsafe driving behaviors (drunk driving, fatigue driving, illegal driving, not wearing seat belts, etc.). ), investigate and deal with violations 14 this year, analyze violations, and fill out the three violations analysis card.

(5) Implement the fire safety work plan of oil production plant 20 12.

Grass-roots teams set up fire fighting files for key fire fighting units such as gas gathering stations and oil pump stations, set up voluntary fire fighting teams for key parts, and organize a fire fighting drill for different emergency fires in key parts every quarter.

Strengthen fire management, eliminate fire hazards, and organize fire safety supervision regularly. Full-time fire safety management personnel in grass-roots departments here conduct daily, weekly and monthly fire patrol inspections, timely declare the replacement of defective fire-fighting equipment, dredge escape routes, and supervise emergency lighting and escape lamps and equipment.

Organize special supervision on fire safety. The safety office in the operation area regularly organizes special supervision and spot checks on fire safety. Investigate and deal with hidden dangers and violations, severely punish them, and follow up and urge rectification. This year, the Office of Safety Supervision carried out four special inspections on fire safety, investigated and dealt with 365,438 hidden dangers and 2 violations, all of which have been rectified and implemented.

(6) Implement the emergency safety work plan of oil production plant 20 12 to improve the emergency handling capacity of the operation area.

According to all kinds of emergencies that may occur within the jurisdiction of the operation area, formulate and improve the emergency plan as a guiding basis for emergency response and drills. A leading organization for emergency rescue in the operation area was established, and an annual emergency drill plan was formulated. Organize an emergency drill every quarter in strict accordance with the plan, conduct emergency training for personnel at all levels in the operation area every month, and organize desktop deduction. Grass-roots teams should establish their own emergency organizations at all levels under the overall deployment of the operation area, and organize emergency training and emergency drills every month, with no less than 12 emergency drills every year. Up to now, all four quarterly emergency drills in the operation area have been completed in a down-to-earth manner. Through actual combat drills, the response ability of emergency command and leadership institutions and on-site rescuers in the operation area has been exercised, and it has also been verified that the emergency rescue institutions in the operation area are fast, efficient and can stand the test of actual combat.

(7) Implement the implementation plan 20 1 1-20 12 for upgrading the training demand matrix of oil production plants, and fully implement the training demand matrix.

According to the Guiding Opinions on the Implementation of HSE Post Training Demand Matrix for Employees in Oilfield Company and the relevant notification requirements of the oil production plant, the operation area actively carries out communication, extensively collects information from all parties, and draws lessons from the experience of matrix pilot units such as Anshan and Technical Work to solidly carry out the job training demand matrix for employees in the operation area.

2. A matrix work implementation leading group was set up in the operation area. Under the unified leadership of the leading group, a matrix implementation team headed by manager Li, who is in charge of safety, was established, and the implementation team was responsible for the concrete implementation of the matrix. In view of the shortcomings and weak links in safety production management, the operation area carries out monthly HSE assessment and analysis in a targeted manner to improve risk management and control capabilities. In the operation area, the safety directors of all departments shall be called to hold monthly safety meetings and safety committee meetings every month, and the monthly summary report on the supervision of oilfield companies and oil production plants shall be carefully studied, focusing on the "three violations" and hidden dangers of the notified oilfield companies and oil production plants. And distribute the contents of the regular meeting to all offices and teams, and organize grassroots employees to analyze and study in depth. Circularly share the safety experience in accidents and cases, and implement inductive leadership according to the principle of "focusing on things around us, focusing on things in the factory, and selecting typical accidents of oilfield companies and other oilfields". We will circulate from the first person in charge of safety to the person in charge of each office in turn to share safety experience, which reflects the transformation of leading cadres from "attaching importance" to "emphasizing reality" and functional departments from "participants" to "responsible persons".

Four. Implement the regulations on administrative penalties for those responsible for production safety accidents and environmental incidents of the Group Company.

In order to standardize and unify the standards for investigating and handling personnel responsible for production safety accidents and environmental incidents. According to the nature and level of the accident and the division of responsibilities of direct responsibility, main responsibility, main leadership responsibility and important leadership responsibility, the responsibilities of operation service personnel, management personnel, business leaders at all levels and main leaders shall be strictly investigated. In the operation area, efforts should be made to publicize the Provisions on Administrative Punishment of Persons Responsible for Production Safety Accidents and Environmental Events. The safety office in the operation area organized safety management personnel at all levels to publicize and study twice, with 46 participants, and distributed the regulations to all offices and oil production teams to organize and implement the study, so that the publicity could be implemented to teams and individuals.

Five, to carry out occupational hazard detection, strengthen the prevention and control of occupational diseases.

1. Organize the detection of occupational hazards in the workplace. In August, organize occupational hazard detection in 10 workplaces that may cause occupational hazards, and post the detection results on the bulletin board of workplaces.

4. In the operation area, the newly revised Law on the Prevention and Control of Occupational Diseases was studied, publicized and trained twice, and 54 people were trained.
