The internship certificate should be issued by your internship company. Before the internship, the school will send a blank internship certificate data sheet to stamp. After the internship, the internship unit will fill it out and finally stamp it.

The validity period of nurse practice registration is 5 years. If a nurse needs to continue practicing after the expiration of the validity period, it shall apply to the original registration department for renewal of registration 30 days before the expiration of the validity period. If you don't extend the validity period, the procedure is extremely troublesome, and you must cancel the registration first and then re-register.

Re registered nurses shall submit the following materials:

1. Approval Form for Nurse Practice Registration;

2. The identity certificate of the applicant;

3. Proof of academic qualifications and clinical practice of the applicant in professional study;

4. Proof of nurse qualification examination results;

5. Physical examination certificate of the applicant within 6 months issued by a general hospital above Grade II;

6. Employment certificate of medical and health institutions (labor contract);

7. Two recent two-inch bareheaded frontal half-length color photos;

8 medical and health institutions in charge of health administrative departments to develop secondary and above teaching, general hospitals for more than 3 months of clinical nursing training and assessment of qualified proof.
