1. "Parent-child relationship statement" signed by the baby's parents.
2. Indirect evidence of the baby's birth. Collateral evidence is the birth certificate issued by the midwife (with a copy of the midwife's ID card), or the relevant certificate issued by the residents' (villagers') committee where the baby's parents are registered.
3. DNA paternity issued by a statutory appraisal institution.
4. The original ID card of the baby's parents and the verified copy of the household registration book.
Born in a medical and health care institution, and failed to apply for the "Birth Medical Certificate" for more than 3 months after birth due to special reasons, the "Birth Medical Certificate" shall be issued for the first time.
