For babies born in non-midwifery technical service institutions, the district-level "Birth Medical Certificate" management institution shall issue the "Birth Medical Certificate", and the parents of the baby shall provide all the following supporting materials, which shall be permanently preserved by the management institution.

1. "Parent-child relationship statement" signed by the baby's parents.

2. Indirect evidence of the baby's birth. Collateral evidence is the birth certificate issued by the midwife (with a copy of the midwife's ID card), or the relevant certificate issued by the residents' (villagers') committee where the baby's parents are registered.

3. DNA paternity issued by a statutory appraisal institution.

4. The original ID card of the baby's parents and the verified copy of the household registration book.

Born in a medical and health care institution, and failed to apply for the "Birth Medical Certificate" for more than 3 months after birth due to special reasons, the "Birth Medical Certificate" shall be issued for the first time.
