3. Each card reader is limited to one card. If the reader already holds one type of card, he must go through the formalities of returning the card before he can apply for another type of card.

4. Older people, disabled people, low-income people and minors under the age of 18 who are over the age of 65 shall apply for the China library card, which shall be handled in accordance with the Measures for Services for the Elderly and Minors.

(2) Foreign language library card

1. Required information: valid certificate.

2. Application method:

Class A certificate: valid for half a year, with a cost of 5 yuan, a deposit of 200 yuan and a loss of 60 yuan. You can borrow 5 boxes of audio-visual materials (no more than 12) or TV series 1 episode at a time, and the TV series must be borrowed and returned as a whole; At the same time, you can borrow 1 computer books, and the usage fee of each book is 1 yuan, and the total loan materials do not exceed 130% of the deposit.

Class B certificate: valid for a long time, with the cost of 5 yuan and the deposit of 100 yuan. Every time you borrow a box of audio-visual materials, you need to pay 2 yuan damages. You can only borrow 2 boxes of audio-visual materials (no more than 6 pieces) at a time. If there are more than 6 pieces of information in the whole box, 2 pieces are counted as one box, and TV dramas cannot be checked out.

(9) Self-study certificate

1. Required information: valid certificate, 1 photo one inch smaller.

2. Application method: validity period 1 month, management fee 10 yuan; One-time license for half a year, management fee of 50 yuan.

Measures for the administration of reader cards

First of all, the reader's card must be registered regularly. If the period of validity is not specified, it will be re-registered after one year of use and pay the 2 yuan registration fee. A valid reader's card shall be re-registered after the expiration of the validity period, and other fees except fees and deposits shall be paid, and the validity period shall be extended accordingly. Please show me your reader's card when registering. If there is any change in personal data, you need to fill in the Reader Replacement Registration Card and submit it to the staff for change.

Second, readers can apply for a certificate cancellation. When returning the card, the reader's card and other valid personal documents should be presented at the same time, and the deposit receipt of the reader's card should be presented when necessary. The deposit will be refunded in full, and other fees will not be refunded. Chinese periodical borrowing card and third-class art design materials membership card are non-refundable.

Three. Readers who lose their library cards should call or go to the library registration office to report the loss. The loss caused by not reporting the loss shall be borne by the reader himself. After reporting the loss of the telephone, the reader must go to the library to handle the relevant formalities in person within one week. The self-study certificate and membership card of the third kind of art design materials are not submitted, please keep them properly.

Four, the reader card is lost, you can reissue a new card. The computer verifies that there is no book borrowing record, cancels the original card and pays the fee before issuing a new card.

Measures for services for the elderly and minors.

In order to implement the Regulations on Public Cultural and Sports Facilities and other relevant laws, regulations and policies, these Measures are formulated: 1. Preferential service targets: elderly people over 65 years old, disabled people, low-income people,/kloc-minors under 0/8 years old.
