How to apply for a health permit? Health permits are mainly applicable to beauty salons, bathing, catering and accommodation industries. In Suzhou, you have to go to the local industrial and commercial department to pre-approve the name of the unit, and then go to the health supervision office to apply for a health permit. You should bring a copy of the Notice of Name Pre-approval, a copy of the legal representative's ID card, a floor plan, a real estate license rental agreement, a copy of the employee's health certificate, a health management system, a one-year effective inspection report of major products, and a registration form for basic sanitation facilities. This is my job.

How to apply for a health permit in Qinzhou? Do you go to the investment service hall? Go directly to the window of the Health Bureau ... He will give you a bunch of forms ... You need to add an application form ... square bitmap, floor plan, health system, provide industrial and commercial name verification, etc ... and then take it back to the staff for inspection ... After passing the exam, ask him to visit the site ... He will express his opinions ... After you change it ... After passing the exam, you will wait for the certificate ... This will definitely cost more than finding an agent. The health bureau is on the downhill side of the second bridge!

Does the cold storage need a hygiene license? According to the different nature of use, cold storage can be divided into three categories: production cold storage, distribution cold storage and life service cold storage. Productive cold storage is an important part of food processing enterprises; Distributed cold storage is generally built in big cities or land and water transportation hubs and densely populated industrial and mining areas to store food for market supply and transportation; Life service cold storage is used to temporarily store food to meet the needs of life. The cold storage mentioned by the landlord should be properly reviewed according to the specific situation!

Do internet cafes need health permits? Internet cafes belong to public places, but according to relevant regulations, there is no need to apply for a health permit. 1987 The Regulations on Hygiene Management in Public Places approved and promulgated by the State Council stipulates that 28 kinds of public places in 7 categories shall be managed by hygiene licensing system. Because there was no such new thing as internet cafes at that time, it was not included in the management category. 2011The revised Detailed Rules for the Implementation of the Regulations on Hygiene Management in Public Places, which came into effect on March 22nd, did not include Internet cafes, which means that Internet cafes do not need to apply for hygiene licenses in public places.

If Internet cafes deal in food, they need to apply for a food hygiene license.
